1. Access the Individual Registration Form HERE.
2. The Head Coach completes the first section of the form, then is sent to the athlete by hitting “Send to Athlete and Parent / Guardian to sign.”
3. The athlete/parent/guardian then receives an email to complete the second portion of the form and officially sign to the Club.
4. Once the athlete/parent/guardian complete their portion, both the coach and VM will receive the final copy.
5. VM will then have this form on record, and the athlete will be officially signed for the season.
Individual Registration Forms will be sent out to Approved Clubs before the Offer Period.
