1) Screening Disclosure Form (2026-2027)

The Screening Disclosure Form must be completed on an annual basis.

The purpose of the Screening Disclosure Form is to allow individuals to disclose any relevant infractions that may have been committed during the three (3) year period in which their CRC and CARC are valid.

The Screening Disclosure Form can be found on Sportlomo, under the “Certifications” tab on each individual’s account.

Below are the steps to complete the Screening Disclosure Form:

  1. Login to member profile
  2. Click “Certifications”
  3. Click “Upload Credential”
  4. Select “Screening Policy”
  5. Select “2026-27 Screening Disclosure Form”
  6. The start and expiry dates should automatically populate and then click “save” for the form to open
  7. Fill out the form and click save, to complete the form.

2) Criminal Record Check

Current Background Checks are valid for a period of three seasons.

Current Background Checks must have a date of issue on or after August 1st, 2023 in order to be considered valid for the 2025-2026 season.

Current Background Checks

To apply for your Current Background Check, complete through Trustii through your Sportlomo account. This is recommended for all Manitoba residents. Please follow these Trustii / SportLomo directions. Your check will automatically upload to your Sportlomo account and you will be sent a copy.

3) Child Abuse Registry Check

Child Abuse Registry Checks are valid for a period of three seasons.

Child Abuse Registry Checks must have a date of issue on or after August 1st, 2023 in order to be considered valid for the 2025-2026 season.

Child Abuse Registry Check
(Manitoba Residents Only)

Apply for your Child Abuse Registry Check online. Results can be picked up from the Child Abuse Registry Office in Winnipeg or mailed-out to you (see website for mandatory identification requirements for each).

VULNERABLE SECTOR CHECKS ARE NOT EQUIVALENT TO COMPLETING A CARC.

Referees have the choice to either complete the a) Respect in Sport for Activity Leaders modules, or b) the Safe Sport Training modules.
