Screening Disclosure Form
Screening is required of those in “designated categories” (see Volleyball Manitoba Screening Policy) who work closely with athletes and who occupy positions of trust and authority within the MVA. The entire screening process consists of obtaining a Criminal Record Search Certificate (CRSC), a Child Abuse Registry Check (CARC) and lastly, completing an MVA Screening Disclosure Form.
The MVA Screening Disclosure Form is a form that must be completed on an annual basis while an individual’s CRSC and CARC are valid for a period of three (3) years.
The purpose of the MVA Screening Disclosure Form is to identify individuals within the MVA who pose a risk to its members within the three (3) year period in which their CRSC and CARC are valid.
